Logic Model
Project Empowerment Logic Model
The purpose of a logic model is to link inputs to activities; activities to outputs; and outputs to outcomes. “Inputs” are the resources or raw materials that the program has at inception. “Activities” are the operational definitions of the program in terms of core strategies and key processes. “Outputs” are the yield or by-products from activities that confirm that the program has been delivered. “Outcomes” are the measurable benefits that occur at the individual level as evidenced by the program participants. The Project Empowerment Logic Model serves as a blueprint for program implementation as well as a tool to evaluate the impact of our current activities.
